Donation Request Form

In order to plan our giving for the upcoming season, we ask that you submit proposals for your event no later than April 30th.  We will consider all requests based on merit, but those that pertain to cycling or transit related activities will be given the highest priority.

To select or deselect multiple options, hold the "Ctrl" key when clicking.
Please be as accurate as possible and include all available levels of donation.
(i.e. mailers, flyers, signage, radio, T.V., announcements, ect.)
